Evaluations: What to Expect

The evaluation is a collaborative process, that typically takes place within one 60 minute session.   The therapist will gather important information from the caregiver as well as from the therapists’ own observation and use of standardized evaluation tools.  The therapist for your child selects these evaluation activities and tools based upon information received from the caregiver in the medical history form of the patient packet.  Therefore, it is critical that each family complete and submit the child’s medical history form and patient packet prior to the start of the evaluation so the therapist can determine the best approach for your child’s evaluation session. Stepping Stones must receive the signed consent page from the patient packet in order to begin any services with your child.  Please visit the Forms section above to access printable forms that you can fax, email or snail mail into our office prior to the scheduled start of services for your child. Please remember to include a confidentiality note if emailing or faxing your private and personal information.* Example: *Confidentiality Note:  This message may contain privileged and confidential information intended only for the use of the above named individual.  If the reader of this message is not the intended recipient, you are notified that any review, dissemination, distribution or copying of this communication is prohibited.  If you have received this message in error, please immediately notify the sender by telephone and shred the original to destroy it.
